Hess Home Health You don't have to give up the comforts of home when you are sick or in need of special help. Hess Home Health provides home care services, allowing our patients to function at their optimal level while remaining in their familiar surroundings.
Call 608-847-1480 Available Monday through Friday Weekends by arrangement. Hess Home Health brings together registered nurses, home health aides, physical and occupational therapists, and social workers to offer you complete home care. Anyone in the community may call to request services, including family, friends, ministers, social workers, or physicians. The individual's physician must order home care services and approve a plan of care. Meet our Home Health Team! Who is Eligible for Home Health? n Any person who has an unmet skilled need for nursing, occupational therapy, physical therapy, and/or health aide services, as determined by a registered nurse or therapist. n Any person who has physician approval for treatment. n Any person who lives in the covered service areas which include: Juneau, Sauk, Monroe, Vernon, Adams, and Jackson Counties. n Any person who is homebound. Our skilled registered nurses provide: • Medication injections • Tube feeding • Catheter care • Colostomy care • Diabetes education • Wound care • Bowel care • Exercise programs • Assessment of your medical, social and financial needs • Referrals to other community resources • Long-range planning and decision-making • Assistance with advance directives (making end-of-life decisions) • Coordination of insurance benefits Payment Options Include: n Medicare n Medicaid n Private Insurance n Private Pay Availability: Home Health visits can be scheduled between 8:00 a.m. and 4:30 p.m., Monday - Friday and by pre-arrangement on weekends.
