How to obtain your medical records You must be the patient, or the parent or guardian of the patient for whom you seek records. Caregivers may be able to access records if the patient has provided written permission to the provider. An Authorization For Use & Disclosure of Health Information form must be completed. A signed copy should be sent to MBMC Health Information Department. This form may be mailed, fazed or brought to Health Information in person.
