Discharge planning is available for people leaving the hospital or nursing home, as is financial planning to help you meet the cost of your care.

·         Counseling for resident and family members to facilitate adjustment to nursing home placement and illness.

·         Information and referral on financial resources.

·         Information on and referral to community services available to the resident.

·         Advocating on behalf of resident to ensure that Resident’s rights are respected.

·         Assistance with completing Advance Medical Directives.
