Mile Bluff Clinic, LLP is a Medicare preferred provider by virtue of our status as a rural health clinic site. And we are a “deemed” provider for most Medicare Advantage plans.

We accept the Forward Health-BadgerCare+ plans and we are a contracted provider for Atrium and Health Traditions HMO plans, for patients who have chosen Mile Bluff or New Lisbon Clinics for their primary care.

We bill most health insurance plans, and we participate in a number of managed care insurance plans. Our contracted HMO's include Gundersen Health Plan, Health Traditions (Franciscan Skemp), Physicians Plus and Unity. We also are contracted with BCBS PPO Plans, Benesight, Custom Benefits, Midwest Preferred (Midwest Security Gundersen network), TPA Plus, WEA Point of Service (Gundersen network), and WPS Preferred Advantage.

We are a Tricare Standard provider - patients with Tricare Prime HMO will need a referral from their primary care provider to have full benefits at Mile Bluff Clinic.

If we do not participate with your HMO, or if your PCP is not from MBC, please abide by the terms of your insurance and seek care from your own plan providers. Contact your plan administrators at work or call your insurance company.

Mile Bluff Clinic, LLP is not able to guarantee your level of benefit coverage for services. If you have further questions, please contact your insurance plan for resolution. All insurance plans are a contract between the patient and the insurance company. They will be able to assist you in finding a contracted facility or provider.

Special note to BadgerCare+ members:  If in addition to Forward Health, you have an HMO that does not cover here, please use the providers who are covered on your primary insurance, or get a written referral from them to MBC before making an appointment.
